For your first visit:
Please contact your Primary Care Physician for authorization to see a specialist if required by your
insurance.
Please bring your current glasses and a list of your current medications.
Expect to be at our office for approximately 2 hours for a complete exam.
Your eyes will be dilated during your exam. This can make driving difficult for some patients. You might
wish to bring along a relative or friend who can drive you back.
Do not forget to bring your insurance card as we will need a copy of it in order to bill your insurance
company for our services.
Please show up approximately 20 minutes prior to your first appointment to complete the information
forms. You can also download a copy of these forms below and bring them with you when you come.
